Please advise me before commencing a session if for any reason your health or ability to exercise has changed.COVID/NOROVIRUS/OTHER CONTRACTIBLE ILLNESSES :
If you are experiencing symptoms please do listen to present NHS advice, rest and be mindful of others you come in to contact with. Seek medical advice if at all worried.
Please inform me if you become unwell between sessions so I can take necessary action.
GENERAL INFORMATION
You must advise me if you become pregnant. It is advised to wait 6-8 weeks after the birth before resuming exercise, should you have a natural birth. If you have had a C-Section it is advised to wait 12 weeks before beginning or resuming an exercise program. You must inform me of a C-section within the last two years before partaking.
My exercises are safe but, as with all forms of physical exercise, it is prudent to consult your doctor before starting should you have any injuries or medical conditions. These sessions are not a substitute for medical counselling or treatment. If you have any doubts about the suitability of the exercises you should refer to your medial practitioner. I cannot accept any liability for personal injury related to participation in a session if: - your doctor has, on health grounds, advised you against such exercise.
Exercise should be performed at a pace in which feels comfortable for you. PAIN is a warning system and should NOT BE IGNORED. Please inform me immediately if you feel any discomfort during a session. Please also inform me if you felt any discomfort after a previous session.
INFORMED CONSENT
I hereby consent to voluntarily engage in an acceptable plan of exercise conditioning. I understand that no exercise program is without inherent risks and that, regardless of the care taken by my personal trainer, she cannot guarantee my personal safety. I understand that I am responsible for my own safety when exercising with Gill Dawson and I will make myself aware of risks that surround me. I understand that a regular exercise program has been shown to have definite benefits to general health and well-being. I consent to being placed in program activities, which are recommended to optimise said benefits.
I understand that it is my responsibility to fully disclose to my trainer any health issues or medications that are relevant to participation in a strenuous exercise program, inform the trainer if there are activities with which I do not feel comfortable, to cease exercise and report promptly any unusual feelings (e.g. chest discomfort, nausea, difficulty breathing, apparent injury, etc.) to my trainer and to clear my participation in any exercise program with my physician.
I have either provided a medical release from my physician to my trainer or have refused to obtain said medical release fully acknowledging the risks associated with the exercise regimens voluntarily being undertaken with my trainer and have filled out a registration and enrolment form. I am in good physical condition and have no impairments, which might prevent my participation in exercise activities and have been advised to consult a physician prior to beginning this program.
I have been informed that the information that is obtained in this exercise program will be treated as privileged and confidential and will consequently not be released or revealed to any person without my express written consent.
I agree that I will take measures and allow time to ensure that I am fully recovered at the end of my online session before sitting and resting, this includes when a session is cut short by lack of internet connection or distraction. (applicable to any online coaching I receive)
I agree to notify my trainer if I begin to feel unwell within a session online and I agree to stop cool down, allow my heart rate to calm down before I stop to rest.
I acknowledge and represent that I am 18 years of age or older and have read and understand the contents of this document. I have been made fully aware of and understand the potential risks involved in exercise programs. I hereby consent to those risks and freely and voluntarily agree to participate in an exercise program offered by Gill Dawson.
